The following online forms must be filled out, along with a current physical, in order to participate in tryouts or do anything athletically in Klein ISD. Please be sure to read the directions and complete all forms shown. The PDF is the physical form your doctor must fill out. Bring it with you to the physical and give to our trainers. The online forms will automatically be uploaded to our trainers. If you have not already done so, please turn in your completed physical to our trainers by the first day of school.
